The Government of India launched the Community Health Officer (CHO) app to enhance primary healthcare delivery across the country. The app aims to support CHOs in managing health services at Health and Wellness Centres (HWCs) under the Ayushman Bharat programme. It was introduced by the Ministry of Health and Family Welfare.
Features of the CHO App
The app provides digital tools for CHOs to record patient data, track health indicators, and manage referrals. It integrates teleconsultation services with doctors at higher health facilities. The app enables real-time monitoring of health services and supports data-driven decision-making at the primary care level.
Implementation and Coverage
The CHO app is being deployed across all states and Union Territories implementing HWCs. It targets over 1.5 lakh CHOs trained under the mid-level health provider scheme. The app covers services related to maternal and child health, non-communicable diseases, and communicable disease control.
Technology and Integration
The app is compatible with Android devices and uses cloud-based data storage. It integrates with existing health information systems such as the Health Management Information System (HMIS) and the National Digital Health Mission (NDHM). The platform supports offline functionality to ensure service continuity in low-connectivity areas.
Training and Support
Training modules for CHOs on app usage are included in the National Health Mission’s capacity-building programmes. Technical support is available through a dedicated helpdesk and periodic app updates. The government plans to expand training to auxiliary nurse midwives and other frontline workers.

National Digital Health Mission
Launched in 2020, NDHM aims to create a unified digital health ecosystem in India. It assigns unique Health IDs to individuals for seamless access to medical records. The mission integrates multiple digital health initiatives under one platform to improve healthcare delivery and data interoperability nationwide.
Ayushman Bharat Health Scheme
Ayushman Bharat, launched in 2018, includes two components: Health and Wellness Centres and the Pradhan Mantri Jan Arogya Yojana (PM-JAY) insurance scheme. It targets over 10 crore poor families for secondary and tertiary care coverage. It is the world’s largest government-funded healthcare program by beneficiary size.
Primary Healthcare Infrastructure
Primary healthcare infrastructure in India comprises Sub-Centres, Primary Health Centres, and Community Health Centres. It forms the first contact point for rural populations, covering preventive, promotive, and curative services. The infrastructure faces challenges like workforce shortages and uneven distribution across regions.
Telemedicine in India
Telemedicine in India began in the early 2000s and expanded rapidly post-2020 due to COVID-19. It connects rural patients to urban specialists via digital platforms. The government released Telemedicine Practice Guidelines in 2020 to regulate virtual consultations and prescriptions legally.
